They need to use VAR for time added on. That would then add on another 4 minutes which would be deemed as 3 minutes after they have finished looking at it.

Nah...his knee was coming up too. My take is that it was intentional. He was going to stop the ball however he could.

What I cannot understand is given the ref's position how come he couldn't see it? Unless his view was obstructed by an attacker/defender. Or he wasn't watching the most relevant action!
